Potential WBL Employer: Please complete the form below.

If your organization has multiple locations, identify location in Business Name (example Kroger - New Cut).

Employers MUST provide a primary contact person AND a valid email address.

IMPORTANT: An email will be sent from JCPS Forms ([email protected]) requesting your review and signature on the JCPS WBL Employer Compliance Form (required for JCPS students to participate in a JCPS Work-Based Learning program).

If you do not receive an email from JCPS Forms within 2 business days, please check your spam/junk mailbox.
